Limb Salvage Surgery
Tumours in bones and around joints is a very painfull and difficult condition to treat. The Tumours cause pain, swelling, limitation of joint movements and difficulty in activities of daily life. Tumours can be benign( localised tumours ) or Malignanat ( which spread to other parts of body. The Malignanat tumours are dangerous as most of them are untreatable. The only treatment option for Malignanat tumours is Amputation of the involved limb. Unfortunately most of the malignanat bone tumours occur in young patients. This life and Limb threatning condition is generally considered hopeless. But now with our vast experience in joint replacement,we have started offering them " Limb Salvage Surgeries" . This not only saves the limb but gives a permanent cure to the disease also. Limb salvage surgery means the tumour, the involved bone and the adjoining joint, all are excised in one piece. The whole segment is then replaced with a custom made artificial joint. With the help of Onco surgeon ,the tumour is removed along with a healthy margin of tissue all around so that there is no chance of recurrence. In children teenagers the growth of the affected limb stops due to damaged epiphysis ( growth plate ). To compensate for this we use expandable implants. The lenght of this implant is increased every 6 months, so that there is no shortening of the limb. Till now we have done 25 Limb Salvage procedures including Hip replacement, Knee replacement , Total Femur Replacement, Elbow replacement , Shoulder Replacement and Hemipelvis Replacement. Most of our patients are teenagers and are doing well.

SHOULDER PAIN SHOULDER PAIN Pain in shoulder is a very frequent presentation in routine orthopaedics OPD. The origin of pain can be from Cervical Spine, from Chest, from Abdomen and from Shoulder itself. As a clinician, its our responsibility to make a proper diagnosis before treating the patient. The causes and presentations of actual shoulder pain vary according to the age group. SHOULDER PAIN IN YOUNG There are two types of young patients – One who are office workers (sedentary lifestyle) & Second – People participating in sports activities (Athletes) Office Goers: These patients come with pain starting from base of neck and spreading to both shoulders. Unfortunately, most of them are being treated as Cervical Spondylosis patients. Infact they have muscle imbalance problem which can be easily treated by simple exercises which patient can do even in office. Athletes: Athletes participating in over head spots like Tennis, Swimming, Cricket…. etc. come to us with shoulder pain. This type of pain presents when the athlete rise to throwhit something over his head. It is called “Dead Arm Syndrome”. The reason behind the pain is repetitive trauma to joint capsule in a particular position. It causes scaring and tightening of one area of the joint leading to abnormal movements. They can be treated by physiotherapy and modification of activities in early stage. Unfortunately many of these patients either compromise with their performance level or come so late that surgery is required.

SLAP tear : Nemesis of throwing athlete With increasing popularity of cricket in India more and more patients need orthopaedic advice regarding shoulder problems. Most common complaint of this group is difficulty in throwing . Onset is usually insidious. It starts with mild ache and discomfort generally taken casually. The speed and accuracy of throw starts falling gradually and finally ends with ‘dead arm’. Studied extensively in baseball pitchers , the pathology is a tear in the superior labrum leading to partial detachment of long head of biceps ( Type 2 SLAP tear). SLAP = Superior Labral Anterior to Posterior tear. Why this happens in throwing shoulder : With repeated throwing the posterior capsule of shoulder gets thickened and inelastic. This starts pushing the humeral head superiorly during abduction and external rotation ( throwing position of shoulder). It leads to shearing stress on superior labrum with every throwing action ultimately causing SLAP . How to diagnose : SLAP is till now considered an arthroscopic diagnosis because there is no specific clinical diagnostic test for it and MRI reporting for shoulder is still evolving at most centers. With careful history, thorough examination, high index of suspicion and good interaction between the surgeon and radiologist we can give our patients an accurate diagnosis and hence prognosis.
